Fill out your Medical Data Form
Use a pencil for easy updates. Erase and correct as needed. -
Prepare the Form
Fold the completed form in half, then into thirds. Make sure your name and the MICS logo are visible on the outer panel. -
Insert into the Carrier
Slide the form into the neon MICS carrier and fold the internal tab to secure it. -
Apply Inside Your Helmet
Stick the adhesive-backed carrier to a firm, dry surface inside your helmet. Can be placed beneath Velcro foam pads. -
Add the Reflective Decal
Attach the EMS identifier decal to the left rear exterior of the helmet for quick recognition by emergency personnel.
